RESUMO

Objective:As a newly emerging thing, the construction and operation management of research wards are still being explored. According to the previous practice, this study summarized and shared the key points of a demonstration research ward in Beijing, and provided a reference for the development of domestic research wards.Methods:Focusing on improving the efficiency and quality of clinical research, this article summarizes and shares the experience of research ward construction. In addition, this study explores how to maintain the high-quality sustainable development of research wards from the aspects of improving core competence, system construction, and talent training.Results:Professional teams, innovative operation modes, as well as intelligence and informatization could improve the quality of clinical research. Besides, the improvement of core competence, talent training, and policy support ensure sustainable development of research wards.Conclusions:As clinical research platforms, the development paths of research wards need to be clarified further. The standardized construction and sustainable development of research wards can effectively improve clinical research capability and promote the transformation of scientific achievements.

Objective:To explore the clinical effectiveness of an automatic computer-aided diagnosis(CAD) method in benign and malignant breast masses discrimination.Methods:The ultrasound images of 539 patients from the Second Hospital of Harbin Medical University and the Second Hospital of Hebei Medical University between 2012 to 2019 were analyzed retrospectively. According to the fifth Breast Imaging Reporting and Data System (BI-RADS), four breast radiologists first sent the case into a BI-RADS category with the original ultrasound image. Then with the CAD result, radiologists gave a category again. Pathology results and clinical data were not available to the radiologists during the diagnosis process. The histological and follow-up results were used as the gold standard. The accuracy of CAD automatic classification, radiologists′ diagnosis before and after CAD application were compared using the ROC curves. The accuracy, sensitivity and specificity of the diagnosis were also calculated.Results:The classification algorithm has a good performance in benign and malignant breast masses discrimination.When the cutoff value was 0.495, the accuracy, sensitivity and specificity were 0.878, 0.868 and 0.886 respectively. When the cutoff value was 0.203, the sensitivity and specificity was 0.981 and 0.337 respectively. With the CAD method, the radiologists improved their diagnostic performance. The total area under the ROC curve for the four radiologists increased from 0.775 to 0.871( P<0.001). The total sensitivity increased from 0.786 to 0.842, and the specificity increased from 0.681 to 0.813. Conclusions:The automatic classification algorithm in this study provides quantitative reference for doctors′ diagnosis. It has the potential to improve junior radiologists′ diagnostic performance in differentiating benign and malignant breast masses.

Objective:To verify the clinical value of the good outcome following attempted resuscitation (GO-FAR) score in predicting the neurological status of patients with in-hospital cardiac arrest (IHCA) in the Chinese population.Methods:The clinical data of patients with IHCA who were admitted to the Zigong Fourth People's Hospital from January 1 to December 31, 2020 were retrospectively analyzed. Used Glasgow-Pittsburgh cerebral performance category (CPC) score 1 point as the end point, the subjects were divided into 4 groups according to the score: ≤ 0 group, 1-8 group, 9-20 group and ≥ 21 group. Taken the group which GO-FAR score ≤ 0 as the reference group, the odds ratio ( OR) of the other three groups compared with this group was calculated. The receiver operator characteristic curve (ROC curve) was performed to evaluate the predictive value of the GO-FAR score in favorable neurological outcome. A calibration curve was drawn for the Hosmer-Lemeshow test to analyze the degree of calibration of the GO-FAR score for predicting good neurological outcome. Results:A total of 230 IHCA patients were enrolled in the study, including 130 males, aged 74 (65, 81) years old, and 23 case (10.0%) had good neurological prognosis. There were statistically significant differences in GO-FAR-related variables, including age, a normal neurological function on admitted, acute stroke, metastatic cancer, septicemia, medical noncardiac admission, hepatic insufficiency, hypotension, renal insufficiency or dialysis, respiratory insufficiency, pneumonia, etc (all P < 0.05). Taken the GO-FAR score ≤ 0 group as the reference group, the OR values of good neurological prognosis in the GO-FAR score 1-8 group were 0.54 [95% confidence interval (95% CI) was 0.17-1.53, P = 0.250], 9-20 group were 0.17 (95% CI was 0.02-0.67, P = 0.009) and ≥ 21 group were 0.25 (95% CI was 0.05-0.85, P = 0.025). The area under the ROC curve (AUC) of the GO-FAR score for predicting favorable neurological outcome in IHCA patients was 0.653 (95% CI was 0.529-0.777, P = 0.015) and there was no significant difference in Hosmer-Lemeshow test ( P = 0.311). All these suggested that there was no significant difference between the predicted value and the actual value. Conclusions:GO-FAR score can be applied to predict neurological prognosis of IHCA patients in Chinese population. It can help clinicians to predict the prognosis of cardio-pulmonary resuscitation (CPR) and propose critical recommendations in treatment for these patients or their families.

Objective:To study the willingness of county-level hospital physicians within a county medical alliance(alliance)to participate in county-township human resource integration for medical institutions, for furthering the development of such integration.Methods:Based on a discrete selection experiment, a questionnaire was designed for the selection of the work attributes that affect the participation of county-level hospital physicians in such an integration. On-the-job physicians from four county-level hospitals in a prefecture level city were selected by multi-stage stratified random sampling method as the survey objects, and a field questionnaire survey was conducted from November to December 2021. The mixed logit model was used to analyze the preference of physicians in county-level hospitals for five types of work attributes(work unit, monthly income, seniority required for professional title promotion, training opportunities, serving as middle-level and above administrators), the relative importance of each work attribute, and marginal willingness to pay with regard to their participation in the integration.Results:A total of 172 valid questionnaires were collected. When county-level hospital physicians participate in county-township human resource integration for medical institutions, their statistically significant job attributes and level preferences included monthly income, working unit of county-level hospitals, 7 years for professional title promotion, and more training opportunities ( P<0.001). The relative importance of monthly income ranked the highest among the five categories of work attributes, up to 55.55%. Conclusions:When physicians in county-level hospitals participate in the integration of county-township human resources, the most important was their monthly income, and economic incentives could enhance their willingness to participate.

Objective:To evaluate the clinical effect of Chaihu Shugan Powder in the treatment of liver qi and spleen deficiency syndrome on cancer related depression (CRD).Methods:A total of 63 patients with cancer related depression from our hospital during September 2018 to October 2019 were divided into observation group (42 cases) and control group (21 cases) according to the ratio of 2 :1. The observation group was applied with Chaihu Shugan Powder, while the control group was given placebo agent of Chaihu-Shugan with 2% Chaihu-Shugan Powder and 98% dextrin. Both groups were treated continuously for 42 days. We used HAMD24 and Traditional Chinese Medicine (TCM) Symptoms Scale to evaluate both before and after treatment. Results:The total efficiency about depression in the observation group was 71.4% (30/42) while the control group was 14.3% (3/21). The difference was statistically significant ( Z=-4.22, P<0.01). After treatment, The HAMD scales of the observation group was 18.67±3.55 while the control group was 24.71±3.74 ( P<0.05). The total effective rate of TCM syndrome in the observation group was 71.4% (30/42) and that in the control group was 14.3% (3/21). There was significant difference between the two groups ( Z=-4.00, P<0.01). TCM Syndrome Score of observation group was 55.33±17.59 while the control group was 62.04±10.72( t=-2.00, P<0.05). Conclusion:Chaihu Shugan Powder can effectively improve the anxiety and depression of CRD patients with syndrome of stagnation of liver qi and spleen deficiency.

OBJECTIVE:To compa re the effectiveness and safety of three regimens of tiapride ,clonidine and tiapride combined with clonidine in the treatment of tic disorder (TD)in children. METHODS :A sequential collection of 312 children with TD from the outpatient department of West China Second Hospital of Sichuan University were conducted during Jan.-Dec. 2019. They were divided into clonidine group ,tiapride group ,tiapride combined with clo nidine group ,with 104 cases in each group. Tiapride group was given Tiapride hydrochloride tablets with initial dose of 50-100 mg per day ,and the dose was gradually increased to 150-500 mg per day according to tolerance and clinical experience. Clonidine group was given Clonidine transdermal patches ,once a week ,with initial dose of 1 mg each week ,maintenance dose of 1-2 mg each week ,once a week. Tiapride combined with clonidine group was given Tiapride hydrochloride tablets (same usage and dosage as tiapride group )+ Clonidine transdermal patches (same usage and dosage as clonidine group ). The treatment course of 3 groups was 3 months. After the treatment ,they were followed up every 3 months(the following were expressed as 24,36 and 48 weeks after treatment ). Yale global tie severity scale (YGTSS)scores of 3 groups were observed before treatment ,after 4,8,12,24,36,48 weeks of treatment,and the occurrence of ADR was recorded at different follow up time points. RESULTS :Before treatment ,there was no statistical significance in YGTSS scores among 3 groups(P>0.05). After 4,8,12,24,36 and 48 weeks of treatment ,YGTSS scores of 3 groups were significantly lower than those before treatment (P<0.05). After 4,8 and 12 weeks of treatment ,YGTSS scores of tiapride combined with clonidine group were significantly lower than tiapride group and clonidine group (P<0.05),while there was no statistical significance between tiapride group and clonidine group (P>0.05). At 24 weeks of treatment ,YGTSS score of children in tiopride combined with clonidine group was significantly lower than tiopride group (P<0.05),but there were no significant differences between tiopride combined with clonidine group and tiopride group ,and between tiopride group and clonidine group (P>0.05). After 36 and 48 weeks of treatment ,there was no significant difference in YGTSS scores among 3 groups(P>0.05). After 12 weeks of treatment ,the results of P value corrected by Bonferroni method showed that YGTSS score of tiopride combined with clonidine group was significantly lower than those of tiopride group and clonidine group (P<0.016 7), while there was no statistical significance in the difference between tiopride group and clonidine group (P>0.016 7). There was no statistically significant difference in the total incidence of ADR among 3 groups(P>0.05). CONCLUSIONS :Clonidine,tiapride and tiapride combined with clonidine can significantly improve the tic symptoms of TD children with good safety .

OBJECTIVE:To study the current status and influencing factors of medication compliance in children with tic disorder(TD),and to provide reference for improving medication compliance in TD children. METHODS:The questionnaire was designed according to the protection motivation theory. The cross-sectional study was adopted to conduct questionnaire survey among TD children in West China Second Hospital of Sichuan University from Jan. 2018 to Dec. 2019. The structural equation model was established according to the theoretical assumptions,and the maximum likelihood method was used to estimate the model;multiple linear regression analysis was carried out for the factors with significant influence in the single factor analysis,and path analysis and intermediary effect test were carried out. RESULTS:A total of 317 patients with TD were included,the mean age was(8.38±2.54)years,and the mean course of disease was(3.19±2.46)years. Average medication compliance scores was (5.70±1.69),among which 15.1% was low compliance,37.5% moderate compliance,and 47.3% high compliance. Multivariate linear regression analysis showed that comorbidities(β=0.124,SE=0.167,P=0.011),education level of the main guardian(β= 0.236,SE=0.110,P<0.001),quality of life(β=0.399,SE=0.112,P<0.001)and the types of drugs taken(β=0.166,SE= 0.047,P=0.001)were the factors affecting medication compliance of children with TD. Structural equation model analysis showed that severity(β=0.295,95%CI:0.103-0.493),external return(β=0.830,95%CI:0.662-1.002),self-efficacy(β=0.200,95%CI: 0.057-0.353),susceptibility(β=0.220,95%CI:0.084-0.352)and quality of life(β=0.353,95%CI:0.211-0.500)had a direct positive impact on medication compliance. Quality of life mediated between external returns and compliance variables(intermediary effect accounted for 13.9% of the total effect value). CONCLUSIONS:Children with TD have low medication compliance. It is recommended that pediatricians in medical institutions at all levels to manage the medication compliance of patients with TD from the severity,susceptibility,external returns and self-efficacy,so as to improve patients and guardians’awareness of the severity and susceptibility of disease and medication non-adherence,weaken external returns and increase self-efficacy,and ultimately improve medication compliance of patients

Objective@#To establish a CPA-nucleic acid test strip method for rapid detection of Vibrio parahaemolyticus thermostable direct hemolysin (TDH) and heat-labile hemolysin (TLH).@*Methods@#Primers and probes were designed according to the sequence of TDH and TLH virulence genes of Vibrio parahaemolyticus, The optimal reaction temperature and time were established by optimizing the reaction system. The CPA method was compared with the fluorescence quantitative PCR.@*Results@#The optimal temperature and time for the CPA-nucleic acid test strip method established in this study were 60 ℃ and 40 min, which were highly specific to the TDH and TLH virulence genes of Vibrio parahaemolyticus. The TLH And TDH detection limit of 1.8 cfu/mL and 16.0 cfu/mL; consistent with the method of fluorescence quantitative PCR, and has good stability. The positive rates of TLH and TDH genes were 100% (464/464) and 72.6% (337/464) in 464 isolates, respectively. The positive rate of TDH virulence genes in isolates of aquatic products was 7.81 % (10/128). The positive rate of TDH virulence gene in clinical isolates was 97.32% (327/336).@*Conclusions@#The CPA-nucleic acid test strip method for detection of Vibrio parahaemolyticus TDH and TLH genes is rapid, efficient, sensitive, specific and easy to operate. It can be used in the rapid field detection of Vibrio parahaemolyticus.

Objective To study and analyze the rehabilitation effect of self prepared wind relieving antispasmodic Decoction and psychological intervention in the treatment of sequelae of stroke. Methods 100 patients with sequelae of stroke treated in our hospital from February 2015 to August 2016 were selected and randomly divided into the control group and the experimental group, with 50 patients in each group. Patients in the control group were given conventional treatment, given the corresponding blood pressure, blood sugar control drugs, and brain cell protective agents. On the basis of this experiment, the experimental group was given self prepared wind relieving antispasmodic decoction, and psychological intervention was carried out to strengthen communication with patients.The therapeutic effects of the two groups were compared. Results After the corresponding treatment, the effective rate of the experimental group (92.0%) was significantly higher than that of the control group (74.0%),the difference was statistically significant (P<0.05). Before treatment, there was no significant difference in the CRP levels between the two groups. After treatment, the CRP level was(3.1±0.3)μg/mL in the experimental group was significantly lower than that in the control group,the difference was statistically significant (P<0.05). Conclusion The self-made wind relief soup and psychological intervention treatment of stroke sequelae can significantly improve the rehabilitation effect of patients, to a great extent, reduce the level of CRP, with clinical significance of further promotion.

We identified and characterized the full-length genome of a GI.8 norovirus strain CHN/Huzhou/N10 isolated in an outbreak in Huzhou,China.The full-length genome of CHN/Huzhou/N10 was amplified using five pairs of primers which were designed according to the full-length GI norovirus genome sequences published in GenBank database.Multiple alignments were performed using DNAStar,the phylogenetic relationship of CHN/Huzhou/N10 and the representative NoV (Norovirus) strains from each genogroup were assessed using the software MEGA version 6.0.The viral genome of CHN/Huzhou/N10 was 7 740 nucleotides in length,which was consist of three ORFs spanning 5-5 404 nt (ORF1),5 388-7 019 nt(ORF2),and 7 019-7 660 nt (ORF3),respectively.Phylogenetic analysis based on polymerase and capsid sequences VP1 and VP2 region indicated that CHN/Huzhou/N10 belonged to GI.8 genotype.The amino acid sequence analysis of the VP1 region showed that CHN/Huzhou/N10 had 16 mutations compared with the representative strain Boxer/2001/US,12 of these variations were located in the P2 subregion.Moreover,a single amino acid change (T347S) occurred at histo-blood group antigen (HBGA) binding site Ⅱ and another single amino acid change (T397E) occurred at HBGA binding site Ⅲ.In this study,the first full genome of norovirus GI.8 isolated in Huzhou,China was extensively characterized.The data would be helpful not only for the epidemiology study,but also for the diagnostic tool development and effective vaccine design in the future.

Objective To investigate the epidemic features of pediatric allergic rhinitis (AR) in Chongqing City .Methods The children aged 2 to 12 years old from 11 kindergartens and primary schools in Chongqing City were randomly extracted as the study subjects .The AR questionnaire was designed and conducted the epidemiologic survey and statistical analysis .Results The self-reported AR prevalence rate among children in Chongqing City was 28 .5% (334/1170) .The clinical diagnosis rate was 18 .1%(212/1170) and definite diagnosis rate was 17 .3% (202/1170);intermittent AR accounted for 36 .6% (74/202) and persistent AR accounted for 63 .4% (128/202) .Among the nasal symptoms in AR children patients ,nasal itch occupied the highest proportion (70 .7% ) ,followed by nasal discharge(59 .3% ) ,nasal obstruction(56 .3% ) and continuous sneezing (45 .5% );complicating ocular symptoms accounted for 34 .1% ,those with cough symptom accounted for 56 .9% .Eczema was the most common concomitant dis-ease for children with AR(37 .7% ) ,followed by allergic conjunctivitis (27 .5% ) ,nasosinusitis(26 .9% ) ,epistaxis(25 .1% ) ,asthma (19 .2% ) and secretory otitis media (6 .6% ) .Conclusion The self-reported morbidity rate of children AR in Chongqing City is higher and persistent AR is more common than intermittent AR .

Objective To investigate the relationship of serum adiponectin (APN) gene promoter region single nucleotide polymorphism (SNP) frequency and type 2 diabetes mellitus (T2DM).Methods 120 cases of T2DM were divided into Yin Deficiency Syndromes(n=42), Yin hot (n=38), yin and yang (n=40) and 50 cases of normal volunteers were select as the control group. The diponectin gene (aPM1) promoter polymorphisms of each group were detected with polymerase chain reaction amplification (PCR).Results Serum APN Yin hot levels in patients with T2DM (6.98 ± 1.23 μg/ml) were lower than Qi and yin (2.55 ± 0.78 μg/ml) and yang group (3.48 ± 0.22 μg/ml) (P0.05).Conclusion T2DM patients Yin hot type inherent relationship with aPM1-11377G/C polymorphism, aPM1-11377G/C polymorphism may reflect R2DM Disease Syndromes typing a certain extent, and by influence insulin resistance in patients with arterial plaque and serum APN levels, thereby affecting T2DM disease occurrence and progression.

This article was aimed to rapidly analyze chemical composition in A loe Barbadensis Mill, and to compare the chemical composition of commercial aloe vera medicinal materials with that of fresh aloe yellow exudate. An opti-mized liquid chromatography-mass spectrometry-ion trap-time-of-flight (LCMS-IT-TOF) method was applied for the analysis of commercial aloe vera medicinal materials and fresh aloe yellow exudates. The Agilent TC-C18 column (4.6 í 250 mm, 5 m) was used. The gradient elution was a solvent system of water(A)-methanol(B). ESI source was operated in both positive and negative ion modes. The results showed that chromones, pyrones, naphthalene deriva-tive, anthrones and anthraquinones were separated successfully, 30 compounds were characterized by the comparison of characteristic MS/MS fragment ions data with the literature. The diagnostic fragmentation patterns of different chemical compositions were also discussed on the basis of EST-IT-TOF MS data. It was concluded that the chemical composition of commercial aloe vera medicinal materials were significantly different from that of fresh aloe yellow ex-udate in terms of types and contents: the former one mainly contains isoaloeresin D and aloin, and few aloesin; but the latter is mainly composed of aloesin and aloin, and the content of aloesin is the highest. The LCMS-IT-TOF analysis can be used to rapidly obtain rich structural information of different chemical compositions, which improves the efficiency of qualitative analysis of chemical composition, and is of great significance to the quality control, eval-uation and the utilization of A loe Barbadensis Mill.

Objective To observe the analgesic effect of Qilong Toutong Granule and explore its underlying mechanisms of action. Methods Kunming mice were randomly divided into blank group, Zhengtian pill group, ibuprofen group, and high-, medium- and low-dose of Qilong Toutong Granule groups. After completing the prophylactic treatment for 3 days, hot-plate test and acetic acid-induced writhing test were used to observe the pain reaction of mice. SD rats were randomly divided into blank group, Zhengtian pill group, model group, rizatriptam group, and high-, medium- and low-dose of Qilong Toutong Granule groups. After gavaged for continuous 7 days, rats were intraperitoneally injected nitroglycerin (10 mg/kg), and 4 hours later, blood samples were collected from postcava for measuring the levels of plasma CGRP and β-EP by radioimmunoassay. Subsequently, rats were perfused transcardially and the brain tissues containing the trigeminal nucleus caudalis (Sp5C) were achieved for detecting the expression of Fos protein by immunohistochemical method. Results Qilong Toutong Granule significantly raised the pain threshold, reduced the number of writhing response of mice, remarkably down-regulated the levels of plasma CGRP, up-regulated the levels of plasmaβ-EP and inhibited the expression of Fos protein in Sp5C of rats compared with the model group. Conclusion Qilong Toutong Granule has good analgesic effect and its mechanisms on relieving migraine may be through regulating the levels of neurotransmitters or neuropeptides, and inhibiting the activation of Fos pathway.

To prepare recombinant human retinol binding protein 4 (RBP4) by using the baculovirus expression system and to detect its immunogenicity, the fusion DNA fragment of secretory signal peptide SS64 and human RBP4 gene was subcloned into a baculovirus transfer vector pFastBac-dual(pFBd), and the corresponding recombinant transfer plasmid was transformed into E. coli strain DH10bac, after transposition recombinant shuttle bacmid was screened out. The logarithmic phase Sf9 cells were transfected with the recombinant bacmid and then the recombinant baculovirus containing hRBP4 expression box were generated. After amplification of recombinant baculovirus, the recombinant baculovirus seeds were obtained. To express human RBP4, logarithmic phase Sf9 cells were infected with the virus seeds and SDS-PAGE and Western blotting were used to detect and identify the expression. Finally, to prepare a batch of RBP4 protein, logarithmic phase Sf9 cells in suspension culture were infected with recombinant baculovirus seeds and the supernatant was harvested after 120 hours post-infection for purification. Finally for preparation of polyclonal antibody and evaluation of immunogenicity, the recombinant hRBP4 from insect cells and from E. coli were immunized rabbits. Restriction enzyme digestion and sequencing confirmed that the recombinant baculovirus transfer plasmid was constructed correctly, and subsequently recombinant RBP4-bacmid was generated successfully. SDS-PAGE and Western blotting analysis suggested that human RBP4 protein was highly expressed in Sf9 cells with the molecular weight of approximately 23 kDa. The recombinant RBP4 protein could be secreted into the medium efficiently, and the expression level was calculated amount of 100 mg/L. Finally the rabbit antiserum was harvested after recombinant RBP4 immunization, therein the titer of antiserum against baculovirus recombinant RBP4 is 1:100 000 whereas the titer of antiserum against E. coli recombinant RBP4 is only 1:10 000. Overall, human RBP4 was high efficiently expressed successfully with good antigenicity in baculovirus system, and high affinity antiserum was obtained. A solid foundation was laid for the next step of the preparation of human serum RBP4 detection kit.

RESUMO

To investigate the chemical constituents of A. barbadensis, aqueous extract of the plant was subjected to preparative medium pressure liquid chromatography (MPLC). The chemical structures were mainly determined by spectroscopic evidences (UV, IR, HR-MS, 1H NMR, 13C NMR, HSQC, 1H-1H COSY and HMBC) and chemical methods. A new O, O, O-triglucosylated naphthalene derivative, together with two known 6-phenyl-2-pyrone derivatives and four 5-methylchromones, were isolated and identified as 1-((3-((4- O-beta-D-glucopyranosyl)-beta-D-xylopyranosyloxymethyl)-1-hydroxy-8-alpha-L-rhamnopyranosyloxy)naphthalene-2-y])-ethanone (1), 10-O-beta-D-glucopyranosyl aloenin (2), aloenin B (3), aloesin (4), 8-C-glucosyl-(R)-aloesol (5), 8-C-glucosyl-7-O-methyl-(S)-aloesol (6), and isoaloeresin D (7). Compound 1 is a novel naphthalene derivative and named as aloveroside B, compounds 2-3 are isolated from this Aloe species for the first time.
